This is a simple overall health assessment that can help you determine where you are in each aspect of well-being and point to areas where you might want to make changes. It's 100% free.

We'll ask you 10 simple questions. Don't overthink your answers—just select how you feel today. Your answers will be emailed to you.

After you take your free well-being quiz, you'll be provided with your rating via email along with some general feedback recognizing the positive aspects you already have going for you and suggestions to improve your well-being.

If you want more personalized support you have two paid options. You can select Abby, our digital well-being advisor as a self-guided resource to help you access personalized HSA eligible recommendations. When you work with Abby, you'll take a more in-depth assessment that takes about 10 minutes. Or you can select live, virtual and personalized support with an accredited health advocate. When you work with a health advocate you get 60 minutes to allow for a much deeper assessment.

Abby uses your personal insights, for example, age, location, general medical history and lifestyle, and uses this information to search for personalized health provider recommendations. My Well Self works with an extensive database of health providers ranging from Naturopaths to Holistic Nutritionists to Homeopaths. We conduct a detailed intake with each provider that helps us understand what specific health issue each health provider can address along with other details like location, personality styles, and treatment preferences.

It's 100% free to take our well-being quiz. If you choose Abby, our digital well-being advisor as a self-guided resource the service costs $40 one time. If you choose to virtually speak with one of our accredited health advocates the service costs $120 one time.

Yes! We offer monthly 15-minute touch bases led by Abby, our digital well-being advisor for personalized tracking and updates to the original recommendations, which are touch points supported by a Registered Nurse and Health Advocate. This costs $40 per month.

Our health advocates are registered nurses who are trained to conduct a general assessment of your lived experience, collaborate on goal setting, and provide recommendations of care providers for you to see based on discussion.

Whether you choose Abby as your digital well-being advisor or you choose one of our accredited health advocates who are registered nurses, this service may be covered under your health spending account (HSA). We suggest you contact your insurance provider and confirm your coverage.
